I understand that the services I am receiving — which may include but are not limited to hair coloring, chemical treatments, haircuts, perming, facials, eyelash extensions, lash lifts, eyebrow lamination, and tinting — are being performed by a student who is currently in training and may not be a licensed professional. These services are provided for educational and practice purposes, and results may vary.
I acknowledge that some of these services involve the use of chemicals, tools, or equipment that may cause allergic or adverse reactions such as redness, swelling, irritation, discomfort, or injury. I confirm that I have disclosed any known allergies, medical conditions, or sensitivities that may affect how I respond to the services.
By signing this waiver, I voluntarily assume all risks associated with receiving these services and release the student, instructors, and training facility from any and all liability, claims, or damages that may arise. This includes, but is not limited to, injury, adverse reactions, dissatisfaction with the results, or any required follow-up treatment.
Whether I am receiving services as a model (free or discounted) or a paying client, I understand that I am doing so at my own risk.
